Excerpt from A Statistical Inquiry Into the Nature and Treatment of EpilepsyThe science of medicine is to be advanced by the careful collection of well-recorded facts, rather than by general statements or unsupported assertions. No inquiry thus conducted with scientific precision can fail to be without value, and to add a mite to that store of positive knowledge from which must emanate all hopes of progress for the healing art. Our acquaintance with the nature of epilepsy is as yet in its infancy, and although much valuable practical information has been put on record regarding this disease, it is believed that the following contribution may not be useless in either confirming or questioning previous conclusions.The clinical aspects of epilepsy are especially difficult to investigate with exactitude. The physician, as a rule, is not himself a witness to the chief phenomena characteristic of the disease. DeKalb Published collectively in 1884 when the study of Epilepsy was ‘in its infancy’ (19) this work is a series of articles (3 of them), published independently and then combined to formulate a comprehensive overview of the disorder and it’s best treatment as it was then understood.1. An Inquiry into the Etiology and Symptomatology of Epilepsy. 1879.2. An Inquiry into the Action of the Bromides on Epileptic Attacks. 1881.3. An Inquiry into the Effects of the Prolonged Admin of The Bromides in Epilepsy. 1884.Interesting enough, and quick enough to read, these little snippets of research in time are quite fascinating as it relates to the advances in epilepsy research up to this point – some 130 years later, and more broadly based in understanding the progression of the medical sciences in a broader scope.Opening with a general oversight regarding epilepsy and it’s expected experiences as was witnessed by the author, the reports soon separates into a discourse and statistical expose regarding both Epilepsia Gravior (Grand mal) and Epilepsia Mitior (petit mal).Regarding grand mal epilepsy the author states some very highly regarding generalizations. These include: the general appearance of premonitory symptoms and auras, convulsions, loss of consciousness and post-event fatigue. This segment also fed into the oft repeated and wondered about ‘tongue biting’ degree of incidence. (68.8% - 205) Attacks are also often irregularly intervallic.Petit mal epilepsy is subsequently generalized. Again, the author presents the statistical findings of his study. 38% off all cases studied showed Epilepsia Mitior. The author suggests that most attacks are regularly intervallic. Ultimately the author states that cases were too infrequent to draw any viable generalizations from as it regards petit mal epilepsy and its presentation.Article #2:In this article, the most important thing to bear in mind is that the subject sample is so small that no definitive conclusions can be reached regarding the mollifying effects of Bromides as it relates to the relief of epileptic symptomatology. ‘The numbers are far too limited to found any reliable dictum upon’ (970).The author suggests ideal doses: 30gr up to TID (318) and a subsequent raising or lowering of the dose, dependent upon the patients reaction and age. Of the Bromides it is expressed ‘it is as close an approach to cure as we can ever expect to arrive at by therapeutic means.’ (494) And the primary reason this article seems to have been published is to offer refutation to the common notion that ‘epilepsy was one of the most intractable of diseases’ (516), however both E. Gravior and E. Mitior appear equally retractable under the treatment of Bromides, despite the ameliorative effects not being experienced by all subjects, even those whom differentiate between diurnal and nocturnal bouts of epilepsy.Article #3:This article commences by discussing the potential dangers of overdosing on Bromides (‘bromism’) and the ways in which it manifests in the patient, specifically the physical and mental depression.The author states: ‘All these abnormal conditions, as a rule, disappear when the consumption of the poison is arrested.’ (1033) He, however, gives note to the fact that there are some patients, which he describes briefly, who have been using Bromides for upwards of 6 years without ill effects.Essentially the author states that for all of the negative and positive symptoms which manifest, and which have been called ‘bromism’, there is no clear cut detail as to which the symptoms truly belong – the use of bromides or the pre-condition of epilepsy. The author and researcher is writing from a perspective which clearly seems biased toward the eras pharmocologies, suggesting that the Bromides, used in duration ‘tend toward eradication of the disease itself and the ultimate cure of the epileptic state’ (1188).And now we have neurology… and more astute measures as it relates to psycho-pharmacopeia.
